Rooted & Rising Veterans & Family Program Information
Purpose: To support veterans, transitioning service members, and adult military family members with practical transition planning, wellness education, resource navigation, family connection, and renewed purpose.

- Warm referrals to veteran-serving and community providers
What to expect
- Respectful, participant-led conversation without pressure to disclose
- Practical planning tools and clearly defined next steps
- Information about community resources and qualified providers
- Clear explanation of program scope, schedule, cost, and referral limits
Before your first session
- Confirm the date, time, location, and format.
- Bring relevant questions and non-sensitive documents only when requested.
- Share accessibility, transportation, caregiver, or technology needs.
- Identify one transition, wellness, family, or resource goal.
Accessibility
Accommodation requests are welcome, including mobility access, hearing or visual communication support, service-animal access, transportation barriers, caregiver participation, and technology support for online sessions.
Scope and safety: Rooted & Rising is not a crisis service and does not replace licensed medical, mental-health, legal, or emergency care.
Organizations and partners
Veteran-serving organizations, employers, schools, churches, nonprofits, and sponsors may request private workshops, cohorts, referral partnerships, collaborative events, or sponsored participant seats.
